Disease management programs allow ElevateHealth to standardize care for specific conditions, based on national clinical standards. This allows the disease path to be eased, maximizing the quality of each patient's life and their ability to participate in activities of daily living. The use of Clinical Pathways and other tools can ensure appropriate utilization of services as well as provide patients with the education and self management tools they need for optimal health.


As a provider of  both Skilled Home Health and Personal Care Services at home, ElevateHealth can deliver Disease Management programs under the supervision of a physician through our Home Health Services division, as well as reinforce adherence to disease management protocols by our Personal Care Services providers during ongoing non-medical care, when appropriate.


Disease Management in Home Health

Heart Care

Disease management programs are utilized for many heart conditions including heart failure and hypertension. Other related programs include those for cerebrovascular accident (CVA or stroke), and coronary bypass graft surgery. Remote telehealth monitoring to capture daily weights, blood pressure, or oxygen saturation levels may be used.

COPD Care

Elevatehealth's pulmonary program places emphasis on self-management of symptoms and proactive management to enhance quality of life. National standards are followed. Telehealth monitoring of pulse oximetry and other vital signs is often included.


Diabetic Care

ElevateHealth adheres to the American Diabetic Association recommendations for education and self management. Patients and their families learn to manage and control their diabetes, minimizing complications which result from poorly managed care.


Wound Care

ElevateHealth has partnered with Medline to utilize an interdisciplinary approach to wound care. Standard wound protocols are used based on the type and condition of wound, to ensure consistency in care and improve outcomes.


Orthopedic Care
ElevateHealth adheres to national standards to ensure that patient's recovering from a surgery, such as total hip or total knee replacement, achieve the highest level of function possible. The multidisciplinary team is often led by a physical therapist, with assistance from skilled nursing, occupational therapy, and home health aides.

Disease Management in Personal Care
When chronic conditions affect the daily function of your loved one or put them at increased risk for decline in condition or hospitalization, or Personal Care staff is well-trained in integrative, preventive care and will support the Medical team in promoting activity and behavior to optimize health.
