Who is eligible for home health from ElevateHealth?

• Are you 65 or older?
• Have you been recently hospitalized or had a decline in condition?
• Do you need therapy?
• Have you had a recent fall or feel like you are losing your balance?
• Would you prefer to receive healthcare at home?
• Have you recently been diagnosed with a new illness?

How much will home health cost?

If you qualify for coverage under Medicare, services will be 100% covered, at no cost to you.

Is your Agency licensed?
ElevateHealth operates a Califonia state-licensed home health agency. All skilled services and oversight are provided under this license.
